The Wrathborn are a new Boss enemy faction introduced in Karneval der Albträume. They have a major role in the map's story, as part of the Easter Egg The End Credits. They are led by the Wrathborn Sentinel, who can only be summoned through the use of a Wrathborn's Dark Essence. As Boss enemies, Wrathborn have large health pools and are mechanically different than the Bad Patch. They also will trample over Zombies, Hellhounds and Werewolves, if they get in the way and do not display any sort of remorse for doing so.

According to The Codex, the Wrathborn are meant to serve as enforcers to a higher power and are monitored and controlled by the Wrathborn Sentinel. This Sentinel also has the ability to summon a Reflection of Wrathborn if their Dark Essence has been inserted into its Altar. These Reflections are weaker.
